Please have the following information ready:
- Name of the patient
- Date of birth of the patient
- Main diagnosis / suspected diagnosis
- Relevant secondary diagnosis
- Infection status
- Care status
- Care requirements (1 to 1 care etc.)
- Callback number and, if applicable, name of the responsible (service) physician
Please send us the following documents for the transfer:
- Doctor's letter / transfer report
- Imaging in Dicom format on CD
- Laboratory results
- External medical records / nursing report
- List of medication
- Contact details of relatives
- Contact details of family physician / family dentist
