Social-medical aftercare is a supportive service for chronically ill and seriously ill children and adolescents with a high need for intervention, which is intended to facilitate the transition from the Clinical Department to their own home.
The aftercare facility at the MHH Children's Hospital helps affected families to cope with the new situation created by the child's illness and to integrate the illness into everyday life. Contact is made with the family while they are still in hospital. Discharge is prepared together with the family and the attending physician and care in the home environment is organized and individually coordinated.
Our multidisciplinary team, consisting of paediatric nurses, a qualified social worker/social pedagogue and paediatricians, works according to the case management method. This means that we offer individual and holistic care both in the Clinical Department and at home. We mainly provide advice, but also answer current questions and talk about fears and concerns with patients and parents.
Social-medical aftercare does not replace the tasks of other departments, but organizes, coordinates and accompanies them. Through social-medical aftercare, we offer professional help for self-help, which is intended to relieve and stabilize the family situation and encourage the family to become independent. In addition, inpatient stays should be avoided or shortened.
The aftercare facility at the MHH Children's Hospital cooperates with the Netzwerk für die Versorgung schwerkranker Kinder und Jugendlicher e.V. and the Kinderkrankenhaus auf der Bult. The aftercare facility has been an accredited member of the Bunter Kreis quality association since December 2014.
