LogO - Pilot project for people with geriatric psychiatric illnesses in the event of a change of location/setting

As part of the LogO project ("Pilot project for people with gerontopsychiatric illnesses in the event of a change of location/setting"), a cross-sector, temporary intervention for gerontopsychiatric patients with an imminent change of location/setting is to be established to reduce interface-related complications. The new form of care consists of a multi-professional, predominantly outreach team ("LogO team") made up of nursing staff, a social worker and a physician. This team usually accompanies people with geriatric psychiatric illnesses and their caregivers for up to three weeks before and after a change of location/setting. This includes all conceivable changes of location/setting, e.g. from home or a care facility to a somatic hospital and back, from a somatic hospital to a short- or long-term care facility, from a somatic hospital to a psychiatric hospital (or vice versa), from one care facility to another or from home to a care facility.
The first round to develop the full concept of the project is now being funded by the Federal Joint Committee (G-BA).
On July 15. 2021 the complete application must be submitted - it will be interesting to see!
Project partners: In addition to the Hannover Medical School and the KRH Psychiatry Wunstorf, the Münster University of Applied Sciences, the LWL Clinical Department Güstersloh and the AOK Lower Saxony are supporting us in the planning and implementation of the project.
