Clarification of a transfusion reaction

Required materials

To request clarification of a transfusion reaction, the Transfusion Reaction Request Card is used and sent immediately to the Institute of Transfusion Medicine with the following materials:

the blood components concerned
Pre-transfusion blood of the patient, if available
Post-transfusion blood of the patient
7.5 ml EDTA blood (1 large red monovette)
7.5 ml native blood (1 large white monovette)
Specification of the quantity of the transfused blood component (transfusion reaction card)

Completion of the clarification

The transfusing physician receives a written report on the results of the clarification of the adverse drug reaction.
