Illness on the examination date and the submission of certificates
Withdrawal from an examination is only possible if there is an important reason. The most common reason is illness. This must be documented by a medical certificate within three working days of the examination date (Section 12 (4) of the Examination Regulations).
The medical certificate and the relevant information must be submitted exclusively via the web form below. After "Submit" you will receive an e-mail with a confirmation link - this must also be clicked on within the deadline, as only then will the e-mail to the Office of Academic Affairs be triggered, which then counts as having been received on time! Check your student e-mail inbox immediately after sending!
Please note:
- The certificate must be scanned in color and sent as a PDF file*.
- If the respective physician's office does not issue a digital certificate, a color scan is sufficient.
- The certificate must be signed by the physician and stamped by the practice!
- The medical certificate must state that you are unable to take the examination on the respective examination date!
- If the medical certificate is not received by the Office of Academic Affairs Medicine by the deadline, without a signature and practice stamp or not at all, the examination attempt will be assessed as "failed".
- A maximum of four deregistrations can be entered, for further deregistrations please fill out another form (incl. medical certificate).
- This web form is only valid for examinations and not for regular courses! Please contact the module coordinator directly!

In the event of an upload error, a red error message appears in the upload field and the form cannot be submitted (it is a mandatory field). If this message does not appear, everything went well.

Only the responsible departments and persons involved in the study program organization have access to the data. My data will be used for processing and documenting the deregistration of examinations. My personal data will only be passed on to the responsible offices and persons who need this data to fulfill the contractual and legal obligations or to implement the legitimate interest of the MHH. This includes the employees in the Office of Academic Affairs Medicine and the contractually bound provider of the examination management system, IQUL. Beyond this, your data will not be passed on to third parties. My personal data will not be passed on to a third country or an international organization. My data will be stored by the Office of Academic Affairs Medicine at MHH for as long as necessary to fulfill the purpose for which it was collected.
