Completion instructions

General note: All forms are PDF documents that can be filled out on a PC. You must therefore open the documents via Adobe, not via the browser.

 

Department/Subject/Practice

  • Internal Medicine
  • Surgery
  • "Elective course"

Do not enter any rotations or specific departments such as Gastroenterology, Vascular and Visceral Surgery, etc., but only "Internal Medicine" or "Surgery"
► Elective subject designation as entered in the PJ portal

 

Tertial times

  • must correspond exactly to the times in the MFT overview (see PJ page)

 

Absences

  • Enter specific dates, no distinction between vacation, illness, etc.

 

Clinical Department of the MHH or an institution designated by the MHH for training purposes

  • Clinical Department of the MHH = Departments directly at the MHH, e.g. Center for Internal Medicine, Dermatology, Pediatric Clinic, etc.
  • Facilities or Institutions designated for training = MHH teaching hospital or practice

 

Signature of the physicians responsible for the training

The PJ certificate can be signed either by the head physician or senior physician of the relevant Department or by the PJ representative of the hospital. The signature of one physician is sufficient.

 

Further notes

  • Students must always use the form of the responsible university , i.e. either ours if the tertial is completed at the MHH or a teaching hospital, or that of the host university if the tertial is completed externally. Please note: the external certificate(s) must not contain the logo or letterhead of the host university/teaching hospital. If in doubt, use the blank form from the licensing regulations.
  • Working days are Monday to Friday, so weekends are not absences.
  • Public holidays are days off and not days of absence. 24.12. + 31.12. are normally ½ working days, but company-specific regulations may also apply (e.g. whole day off). In this case, you should note this as an addition on the PJ certificate before signing it.
  • The date of signature may be the last working day at the earliest. If you take absences at the end of a tertial, this can also be before the official end of the tertial. Please note, however, that the tertial period must still end on the last day of the tertial as stated in the tertial overview.
