We are responsible for the care and support of patients who are undergoing or have undergone a heart transplant or have had a ventricular assist device implanted. Patients who have had a lung or heart/lung transplant are also cared for by us.
Our nursing care is based on the MHH mission statement and the nursing concept of Hannover Medical School. It is planned, carried out and evaluated in a patient-oriented manner, taking into account the psychosocial situation.
Our outpatient care team consists of full-time and part-time registered nurses and a medical assistant. Participation in internal and external training is an integral part of our Human Resources.
Our office hours are:
Monday to Thursday from 07:00 - 16:00
Friday from 07:00 - 15:00
One of our tasks is to support physicians in the comprehensive aftercare of our patients. The function of the donor organ and the necessary immunosuppressive therapy are checked at regular intervals so that possible complications in the form of rejection, infections, transplant vasculopathies and neoplasia can be detected at an early stage. Patients with a ventricular assist device also receive a dressing change and a swab of the driveline during their visit. The documentation of all data is an important task in order to be able to map the course of the disease.
We are also available to our patients, general practitioners and other hospitals for telephone inquiries to put them in touch with the physicians responsible at our clinic. We are also responsible for entering patient-relevant data into the relevant databases.

Building K25 is located almost directly on Stadtfelddamm on the university campus. We are waiting for you on floor 2 on the left-hand side.
Please bear in mind that, for organizational reasons, an outpatient visit takes a lot of time. We would therefore recommend that you bring something to eat and drink and perhaps something to pass the time (e.g. a book).
