We would like to ask you to use the following form for inquiries about lung transplantation if possible.

We are aware that the inquiry process is time-consuming. However, it is only through structured registration that we can guarantee prompt and careful processing. We make every effort to process your inquiries as quickly as possible - for this we need your help.

We require the following documents for a complete registration:

• Current lung function
• Blood gas analysis
• CT thorax
• Body height and weight
• A current physician's letter with main and secondary diagnoses
• Landline and mobile phone number of the patient, if possible with a valid e-mail address (for direct contact)

Please upload CT images here in parallel with the request or send us a possible download link with access code in the following form. Further information can be found under outpatient clinics in the checklist for presentation at the appointment.

Optional, but very welcome, are the following additional findings:

• Abdominal sonography
• Colonoscopy
• Skin cancer screening by the dermatologist
• Echocardiography
• Vaccination status

Request via e-mail:

In exceptional cases - such as acute emergencies - inquiries can also be sent by e-mail. The corresponding address can also be found in the outpatient clinics section of our website.
Please note, however, that we only prefer e-mail inquiries in urgent cases.

Once we have received your complete documents, we will respond in writing and get back to you as soon as possible.
