I confirm that I am taking part in the model degree program in Human medicine at the MHH of my own free will in accordance with § 41 ÄAppO . I acknowledge the following circumstances and confirm my agreement with them:

• Participation in the model study program Human medicine leads to a restriction in the possibility of changing the place of study. In particular, the recognition of academic achievements in the model degree program is not guaranteed in the event of a change of university due to the possible differences between the model degree program and other regular degree programs.

Note from the MHH: In accordance with the admission regulations for the model degree program (§ 2), enrollment at the MHH can only be completed upon submission of the above confirmation.

I confirm that I will be able to provide proof of a degree in accordance with § 18 NHG when I start my studies at Hannover Medical School.

Furthermore, I confirm that I am not guilty of any conduct that would make me unworthy or unreliable to study midwifery and that I am in good health to study midwifery and have the knowledge of the German language required to study midwifery.

1. I confirm that I have completed and provided proof of a suitable previous Bachelor's degree or a comparable study program at Leibniz Universität Hannover or another German or foreign university (with appropriate language certificate) at the time I start my studies at Hannover Medical School.
2. I hereby confirm that, contrary to the above paragraph, I am unable to present a degree, but that I have at least 150 credit points by the application deadline. I will submit the corresponding proof of degree to the degree program coordinator within one semester.

I confirm that at the time of commencing my studies at Hannover Medical School I have completed and provided evidence of a suitable previous Bachelor's degree or a comparable degree program at Leibniz Universität Hannover or another German or foreign university (with appropriate proof of language proficiency).
2. I confirm that, in deviation from the above paragraph, I am unable to present a degree, but have proven at least 150 credit points by the application deadline. I will submit the corresponding proof of graduation to the program coordination within one semester.

I confirm that I have obtained and provided proof of a suitable previous (German or foreign) university degree at the time of starting my doctorate/studies at Hannover Medical School.

I confirm that I have acquired and proven a previous (German or foreign) university degree suitable for the subject at the beginning of my doctoral studies or studies at Hannover Medical School.

I confirm that at the start of my ERAMUS/study exchange at Hannover Medical School, I have provided proof of a suitable previous/current (German or foreign) course of study and that I meet the requirements of my country/university for the exchange.
