Documents for the physician's visit

Please bring the following documents with you to your appointment:

1. Referral form (issued for "Treatment according to §116b, Immunology/MHH), new for each quarter
2. Insurance card
3. Doctor's letters/reports, laboratory values, histopathological and radiological findings (CDs are also welcome, if available)
4. Completed questionnaire at first presentation (see below)
5. Vaccination record

In the interest of the other patients, please arrive punctually at the agreed time. If you are unable to keep the appointment, please cancel it in good time.
