To the patient information
If you have already been diagnosed with a rare disease, the next step is usually a referral to a B centre that specializes in your disease. In order to make this referral, we need medical documentation that clearly states your diagnosis.

If you do not yet have a confirmed diagnosis of a rare disease, but merely suspect that you have one, please click here to find out how to proceed.
We require the following information from you for referral to a B center:
- All relevant medical or hospital reports
- Release of your treating physicians from their duty of confidentiality
- If available: All relevant genetic or histopathological findings
- If available: All relevant radiological or laboratory findings
- If available: Reports from other CSEs
You can send us these documents in two ways:
Transmission by post
