Referral of patients

You can use the form on this page to send us a referral request on behalf of a patient. If a rare disease or unclear diagnosis is suspected, the ZSE will then support you with further contact, expert search, diagnosis and/or treatment. In order for your referral request to be successful, we require various medical documents in advance.

Documents required for referral

To refer a patient to our ZSE, we require the following documents from you:

  • Completed and submitted contact form further down on this page (alternatively as a PDF document from the ZSE download area)
  • All relevant medical or hospital reports
  • All relevant radiological and laboratory findings as well as genetic and histopathological findings
  • ZSE patient questionnaire (see download area of the ZSE homepage)
  • Current referral form to the Centre for Rare Diseases at Hannover Medical School

Submission of the required documents

You can send us these documents in two ways:

Transmission by post

Please send your documents to the following address:

Please do not send us any originals, but only copies of your documents.

Transmission by fax

Please fax your documents to:

If you would like to send us CDs with image data in addition to the fax information, we would be pleased if you would then send us the collected documents by post - and not one part by fax and one part by post.

 

Please note: Due to the high volume of requests, incomplete documents (see points 1-4) cannot be processed.

Please understand that, for data protection reasons, incomplete inquiries must be disposed of after three months in accordance with data protection regulations.
