Notes on form 05 - certificate of achievement according to § 48 Federal financial aid (Federal financial aid)

Form 5 must always be submitted to Studentenwerk Hannover by the 5th semester. If necessary, the Studentenwerk Hannover can also request proof for another semester. It serves as proof of achievement according to § 48 Federal financial aid. The form is issued and signed by the Head of Studies and Examinations of the Office of Academic Affairs Medicine (currently Dr. Louzek) on behalf of the Federal financial aid officers of the MHH for Human medicine study programmes (currently Prof. Dr. Mühlfeld and Prof. Dr. Knudsen).

Please complete the form before submission and then submit it PRE-FILLED by e-mail to: Louzek.Christian@mh-hannover.de

2. STUDY DETAILS and
3. please ask the Federal financial aid office for the date of the semester to be checked and
4. ATTENTION: Please submit proof of training in first aid and nursing service to the Student Office BEFOREhand .
