Dear patients!
Dear relatives!

We appreciate your interest in palliative care at the MHH and hope to be able to help you with the information and contacts on our pages.

The main aim of palliative medical care is to improve the quality of life by alleviating physical and emotional complaints as well as providing support in clarifying care. Influencing the underlying disease is no longer the primary focus of our care. However, the parallel implementation of life-prolonging therapies (e.g. radiotherapy or chemotherapy) does not exclude palliative medical support.

Palliative medical consultation service

Palliative care is generally available in all specialist departments and on all wards during an inpatient stay at the MHH.

As a team of different professional groups (physicians, nurses and, if required, other professional groups such as social services, psychology, etc.), we support patients and relatives, as well as the attending physicians and the ward team.

 

Our tasks include, in particular, the monitoring of pronounced physical or mental complaints, clarification of complex care issues or advice on therapy goals and precautionary planning. Care is always provided in close and professional consultation with the actual treatment team.

Talk to your ward team and the physicians treating you if you would like our support. However, you will often also be approached by the physicians treating you about the possibility of co-care. Of course, you can also contact us yourself if you have any questions about co-care. However, it is important for us to provide care hand-in-hand with the ward team, which is why we will also consult with the ward team in the event of a direct request from patients or relatives.

Our palliative care unit

On our palliative care ward, we care for patients for whom, for a variety of reasons (severe physical or mental complaints, complex care situation), palliative medical care on a normal ward in hospital or on an outpatient basis no longer appears to be sufficient.

The palliative care ward is a specialized ward in the hospital with 7 single rooms. Care is provided by a multi-professional and specially trained team in order to provide the most comprehensive medical and psychosocial care possible. We see ourselves as a place of crisis intervention, so that - if justifiable - further inpatient or outpatient care is organized by us for the follow-up.the palliative care unit is used for short-term and time-limited treatment; the aim is to subsequently discharge the patient home, to another (Nursing) Facilities or Institutions or to a hospice.

Inpatient admission:

In particular, patients from normal and intensive care units of the MHH can be admitted, but also patients who are no longer adequately cared for in outpatient care. As a patient or relative, you can also contact the ward if you wish to be admitted.

However, the medical team on the palliative care ward is dependent on medical information in order to clarify whether the palliative care ward is the right place for care and how urgently admission is required. So don't be surprised if we ask to be put in touch with the physician treating you. Sometimes it is easier if the request is made directly via the treating specialist/family physician or via the outpatient palliative or hospice service.

The Palliative Care Support Center Hanover has developed a joint form which facilitates admission to palliative care units and hospices and which we highly recommend to you and the attending physicians.

Outpatient services at the MHH

The palliative medical consultation service in our outpatient clinic is available to all outpatients with chronic, incurable oncological or other diseases. A consultation can take place at any time during the course of the illness, including alongside disease-specific therapy (e.g. chemotherapy). It also makes sense to contact us if you are already attending another outpatient clinic at the MHH and require additional advice and support. Early contact can help to address physical, psychological and social complaints in good time and to plan further care.

 

Consultation hours take place on Fridays from 8:30-11:00 am. Appointments by telephone only.
