The tumor documentation is a part of the Comprehensive Cancer Center (CCC) Hannover (Claudia von Schilling Center) of the MHH.
It was set up in 1981 and since then has acted as the central institution for, among other things, statutory reporting obligations for the entire Hannover Medical School.
The task of tumor documentation is to document the course of disease and treatment of all patients with malignant neoplasms treated at the Medical School and to systematically record them in compliance with all data protection regulations. It is thus an element of cancer registration in Germany, which pursues the goals of increasing the transparency and quality of oncological care and improving treatment outcomes.
The data collected at the MHH is used to support patient care, for quality assurance and certification procedures, as a basis for medical research projects and for reporting to the statutory cancer registry in Lower Saxony.
Areas of work and responsibilities
As a rule, employees are permanently assigned to the individual work areas (Centers, Clinical Departments, Tumor Diseases). This has the advantage that they have a high level of specialist knowledge in the documentation of tumor diseases in their area of responsibility and are very familiar with the structures and processes of the Clinical Departments in their area of responsibility.
In the event of vacation or illness, the employees stand in for each other, but due to the high workload in all areas of work, this cannot be done for routine tasks.

Tumor documentation systems
In the tumor documentation of the MHH, two systems have been used so far with which the requirements could be fully met.
Due to the increased effort in the area of certifications and the need for electronic reports to the state cancer registries, a future-oriented system change took place on January 1, 2018.
From 1981 to 31.12.2017, the "Hannover Information System for Tumor Data (HIT)" was used at the MHH for tumor documentation. This was an in-house development that was continuously developed and adapted. The system was developed and nurtured throughout the entire period by Dipl.-Math. Dr. Gerd Wegener.
Starting with internal oncology, the system was successively expanded to the entire Hannover Medical School by 2014.
As at 31.12.2017, a total of 85,442 tumor patients were documented in HIT. The documentation from HIT was migrated to the new tumor documentation system by 2020.
Since January 1, 2018, the ONKOSTAR documentation system has been used for tumor documentation at the Medical School. ONKOSTAR is a commercial tumor documentation system from IT-Choice Software AG in Karlsruhe(link).
The following has been achieved so far:
- Since the system change, all patients have been fully recorded in ONKOSTAR
- Connection to the MHH hospital information system (HIS)
- Generation of XML files for reporting to the statutory cancer registries in Lower Saxony (EKN/KKN)
- Determination of data and key figures for all DKG certifications at the MHH
- Generation of XML files for the OncoBox
- Remote maintenance by IT-Choice Software AG has been set up
- IT-Choice support portal is available
- Regular updates are provided and integrated into the system at short notice
- The data access concept for MHH employees has been developed and implemented
- Provision of data and evaluations for physicians and Clinical Departments of the MHH.
The tumor documentation, the Center for Information Management of the MHH (ZIMt) and IT-Choice Software AG are working together on further interfaces (e.g. pathology system of the MHH, biobank) in order to be able to transfer relevant data directly into ONKOSTAR.
Requirements for data and evaluations
The aim of the elaborate collection, documentation and recording of oncological data is that it is used by physicians and scientists to improve the care of tumor patients and to increase medical progress.
MHH employees can submit requests for data and evaluations of tumor patients treated at MHH to the MHH Clinical Data Warehouse (ZIMt) or the Head of Tumor Documentation (CCC).
For inquiries to the Clinical Data Warehouse(link), please use the corresponding online inquiry form (Sharepoint>ZIMt>Research and Teaching>Evaluation Requests)(link). A valid data access agreement (link to Sharepoint) must be attached if necessary. If (further) data from the tumor documentation is required, the Clinical Data Warehouse staff will contact the tumor documentation directly.
If the data or evaluation request relates exclusively to tumor registry data, it can also be sent directly to the head of tumor documentation.
Requests must be made in writing/by e-mail (an online request form is in preparation). As a rule, the tumor documentation staff will provide advice on the request. Valid data access agreements with the signatures of the directors of the Clinical Departments involved must be enclosed.
If medical data is to be evaluated on a personal basis, whether for scientific or non-scientific purposes, the MHH data protection officer must approve this evaluation request (form "Data access agreement", field 10(link)).
The following applies to data requests from or data deliveries to persons and institutions external to MHH:
- Patient-identifying data are generally not delivered by the tumor documentation to recipients outside the university (exception: statutory cancer registries).
- All external data deliveries (anonymized data, evaluations (e.g. "feasibility" requests)) require the approval of the Executive Board of the Comprehensive Cancer Center Hannover in addition to the valid data access agreement.
The following services can be offered by the tumor documentation:
- Provision of data and preparation of evaluations for scientific projects
- Research into patient data for the planning of studies
- Provision of basic data for studies
- Linking of study data with basic data
- Descriptive statistical methods (frequency tables, cross tables, survival curves)
- Inferential statistical procedures (chi-square, t-test, logrank test)
- Transfer of data for further analysis (Cox regression) to statistical systems (SPSS, R)

Statistics in the tumor documentation
Tumor documentarians can take advantage of various training opportunities through the Association of German Tumor Centers (ADT ). Internships
The tumor documentation of the MHH offers internships in which the goals, tasks and activities of tumor documentation are taught and practical work with the tumor documentation system ONKOSTAR from It-Choice is trained. These must be mandatory internships as part of the training. The duration should not be less than 12 weeks.
